What We’ve Been Up to This Quarter
As Isaac Navicare entered its final phase of service delivery, our focus this quarter has been on ensuring a smooth transition for clients, strengthening community resources and supporting the sustainability of local referral pathways.
Sustainability and Transition Activities
A significant portion of our work this quarter involved supporting existing clients as they transitioned to ongoing services and supports. Care Navigators worked closely with individuals, families and service providers to ensure continuity of care wherever possible. This included identifying alternative supports, facilitating referrals and providing information to help people continue accessing assistance after the program concludes.
QCOSS Regional Town Hall
The team participated in the Queensland Council of Social Service (QCOSS) Regional Town Hall, contributing to important discussions about the future needs of rural and regional communities. These conversations highlighted ongoing challenges relating to service access, workforce shortages, housing pressures and mental health support, while also showcasing the strengths and resilience of local communities.
Interagency Engagement
Isaac Navicare continued to actively participate in local and regional interagency meetings, including the Isaac Regional Interagency and Sarina Interagency networks. These forums provide valuable opportunities for organisations to share information, strengthen referral pathways and collaborate on solutions to emerging community needs.
Strong partnerships have always been central to the success of Isaac Navicare, and these relationships will continue to support community members long after the service concludes.
Website Resource Development
Recognising the importance of accessible information, we have continued expanding the resources available through the Isaac Navicare website. This includes links to trusted mental health organisations, local service directories, wellbeing resources and referral pathways designed to help individuals and families find support when they need it.
Professional Development
Throughout the quarter, team members participated in ongoing professional development activities covering topics such as trauma-informed practice, workplace wellbeing, cyber safety, emergency response and person-centred care. Continuous learning has been an important part of ensuring we provide safe, responsive and evidence-informed support.
Community Support Activities
Although service delivery is winding down, we have remained committed to supporting community wellbeing through stakeholder engagement, resource sharing and participation in local events. These activities have helped maintain connections and ensure community members remain informed about available supports.
